ADULT CONSENT STATEMENT

Please read this Consent Statement carefully and answer the two questions at the bottom to continue.

This consult form is for adults 18 years and older attending Come to the Table. 

I acknowledge that the team members of Come to the Table, a ministry of Rapha Nation, have voluntarily agreed to pray for me using various inner healing methods. I understand that because Rapha Nation, Inc. is a prayer ministry and not a professional counseling or therapeutic center.  I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the advice of help that I might receive from the team members of this ministry. I understand that if I receive ministry from Rapha Nation, the team is committed to respecting the information disclosed during sessions, but they are not limited to complete confidentiality if someone is harming themselves or someone else. As needed, the information disclosed in sessions may be shared with other Rapha Nation leaders so as to further my total healing process.  This may include establishing future meetings with spiritual mentors for the purpose of learning to set appropriate boundaries for my personal and spiritual growth.  I understand that Rapha Nation, Inc. is required by law to report the following issues: Any intent of a person to take harmful, dangerous, or criminal action against another person or against him/herself, and any act of child or elderly abuse or neglect.  I agree to hold Rapha Nation, Inc. and its team members free from any and all liability, loss or damage of any kind that may arise as a result of assistance and/or ministry that I have received or from my involvement in Come to the Table, a ministry of Rapha Nation. I have read this release/consent of liability.  I understand and agree with it, and I have executed it as a my free and voluntary act.

 FAQs

  • In-person and zoom, this program can be facilitated as a weekend retreat and/or a 14 -17 week program meeting once a week.

  • Come to the Table is developed for individuals who have a passion to start their healing journey and use these healing principles in their own family and community.

    If your desire is to first focus on your own healing, then we encourage you to sign up for an inner healing session on our contact page.

  • Good question. We are looking for women who agree with the following:

    * You have a relationship with God through His son, Jesus Christ.

    * You like to have fun!

    * You have a regular prayer & word life.

    * You want God to start using you to see miracles.

    * God has called you to ministry in your community and/or the nations.

    * You like good food and laughing.

    * You are a woman who loves God.

    * You are going to commit to attend and participate in 85% of classes. The principles taught at the beginning of the class will be instrumental in the fulfillment of the principles at the middle and end of the class. So, if you miss 2 sessions, then you will be asked to wait & enroll in a future program.

    * You want your faith to soar like eagles wings in the supernatural.

    * You are willing to complete a questionnaire and receive ministry via one or more inner healing/deliverance sessions. These are confidential, 1-on-1 sessions.

    * You agree to sit in as an intercessor for one or more inner healing/deliverance sessions by one of the leaders.

  • Right now, Come to the Table is an invitation to woman & men over the age of 18 years old. We hope to offer this to couples and teens in the near future.
